Profile

Provide therapeutic treatments and evaluation of equipment needs as applicable.

Evaluate the home environment and make recommendations.

Initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points.

Establishes plan of treatment and goals

Skills & Requirements

This position requires a bachelor’s degree or higher from an accredited school of Physical Therapy.

Must be a licensed Physical Therapist in the practicing state.

Experience in Home Health preferred.

TB screening.

CPR certification.

A criminal background check and drug screen must be performed before hire.
