Each Plan of Care is created to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels.nResponsibilitiesnEssential Duties and Responsibilitiesnn Providing interpretation of prescriptions, performing an initial evaluation, and establishing a treatment plan with functional and/or work related goalsn Appropriately progressing the treatment and goalsn Periodic and timely written communication to physiciansn Appropriate discharge management and patient followu2010upn Supervision of rehabilitation aides and administrative staff as neededn Supervision of PTAs when applicable per the appropriate state practice actn Documentation: All PTu2019s are responsible for documenting all treatment and completing all written communications in a timely manner, including but not limited to, initial evaluations, progress notes, discharge summaries, daily S.O.A.P. notes, functional status reports, functional capacity evaluations and insurance/plan of care forms as necessaryn Physical Therapists are responsible for establishing and developing professional relationships with Athleticou2019s referral sources including, but not limited to, physicians, PAs, ARNPs, chiros, case managers, etc.nnExpectations:nn Exceptional patient care and servicen Community Services (such as):n n Hosting a booth at a health related fairn Presenting an inu2010service on orthopedic injuries or preventionn Providing coverage to special events such as sport camps, races, tournaments & lecturesn Performing complimentary screenings to the public with appropriate referrals as necessaryn Ability to demonstrate compassion toward patientsn Service orientedn n Physician office visits u2013 inform physicians of our services, locations, quality patient care and philosophy to be the best providern Accompanying patients to their physician visits (patient visits)n Interoffice communication and follow upn Education: Staff inu2010services, community inu2010services, attendance at continuing education courses, and followu2010up summary of all continuing education courses to staff membersnnAdditional Duties and Responsibilitiesnn Maintain state licensen Complete all compliance standards as requestedn Energetic and a team playernnQualificationsn Education and Experience: The requirements listed below are representative of the knowledge, skill, and/or ability required.nn Degree from an accredited Physical Therapy Programn Current Professional licensure as a Physical Therapistn Current CPR CertificationnnEssential Qualifications: nPhysical Demands: nn Specific vision requirements include the ability to see at close range, distance vision, peripheral vision, depth perception, and the ability to adjust focusn While performing the duties of this job, the employee is regularly required to speak and understandn Possess the ability to fulfill any office activities normally expected in an office setting, to include, but not limited to: remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)n Frequently required to stand, walk, sit, use hands to feel, and reach with hand and armsn Occasionally lift and/or move up to 20u201025 poundsn Fine hand manipulation (keyboarding)nnWork Environment: nn Internal officen The noise level in the work environment is usually lownn nAthletico provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
