JOB SUMMARY: In accordance with our mission and core values, the licensed physical therapist will evaluate and treat patients with neuro/musculoskeletal movement disorders and skin integrity problems utilizing specialized knowledge, judgement and therapeutic skills to return the patient to the highest functional level possible.  The PT will develop treatment plans ensuring multi disciplinary team involvement to meet established goals that follow practice guidelines, regulatory standards and department procedures.


1.  Evaluation/Assessment

Performs evaluation/assessment using critical judgement and clinical decision making.

Accurately documents and interprets findings from evaluation/assessment.

Coordinates objective test procedures, patient history and subjective information.

Understands the importance of physiologic and social information provided in the patient's record and its impact on treatment and expected outcomes.

Provides ongoing evaluation/assessment.

Ensures completion of the Initial Evaluation and Discharge Summary according to department and hospital policy.

Maintains patient progress notes according to department and hospital policy.

 

2.  Establishes and maintains plan of care

Utilizes information about patients' pre admit/pre occurrence status to establish goals and develop treatment plan.

Develops physical therapy treatment plan to support the patient’s goals and is reasonable given the identified problems or deficits.

Evaluates and documents the patient’s response to the treatment plan and makes changes as appropriate.  Alters FIM scores as appropriate.

 

3.  Maintaining Patient Treatments

Obtains or verifies all physician orders for procedures/therapies.

Ensures patient’s informed consent of procedures and therapies.

Assures the presence of appropriate supplies/equipment to perform ordered procedure/therapies.

Performs or assist in therapeutic treatment or procedures according to protocol, standards and department policy.

Performs ongoing comprehensive patient assessment and documentation during therapeutic procedures.

Evaluates and documents the patients response to therapy through progress or lack of progress.

Monitors the patient care record for information on the patient's condition, discharge plan and other multi -disciplinary care givers notes that are pertinent to the ongoing physical therapy being provided.

Coordinates and documents communication between patient, patient’s family, and the multi disciplinary team to support the treatment plan.

Ensures effective and wise use of resources, monitoring for cost containment.

 

 

4.  Teaching

Assesses the patient’s and significant other’s knowledge base, ability and readiness to learn based on their developmental age.

Identifies and utilizes multi disciplinary team members to provide comprehensive education for patient, self and others.

Incorporates teaching principles and methods appropriate to knowledge base, ability and readiness to learn and based on developmental age.

Demonstrates knowledge in area of teaching and available resources for patients, student interns and staff education.

Documents ongoing or completion of patient instruction/education in the patient record.

Documents student intern or staff education as per facility and department policy.

Evaluates the teaching process.

 

5.  Leadership

Participates in peer review.

Provides learning opportunities and contributes to the orientation of team members, per diem personnel and students.

Identifies and implements educational opportunities for the staff.

Serves as a clinical instructor and role model for new employees and students.

Utilizes conflict resolution to promote a cohesive team.

Delegates tasks based on patient load, staffing, patient acuity and departmental needs.

 

 

6. Supervisor/Senior Staff

Participates in the evaluation process of other team members.

Participates in counseling and coaching of employees.

Oversees the delivery of care, including day to day staffing adjustments.

Delegates tasks and assignments according to patient acuity, staffing and patient load.

Ensures exchange of information with appropriate team members within the department or outside of the department is ongoing and timely.

Serves on or recommends staff members to serve on hospital wide task forces or committees.

Provides feedback and information to department director for assessing and projecting material and capital needs.

Systematically supports and empowers staff to make effective decisions that improve service and customer service. 
 
 ','!*!MINIMUM QUALIFICATIONS: 
 
Education:  Current Physical Therapy License in Arkansas
 
Experience: Must have two years experience in acute care therapy or hospital based Outpatient Therapy and demonstrated leadership.
           
Physical Requirements: This job requires, at various times, the following physical activities; climbing, balancing, stopping, kneeling, crouching, reaching, standing, carrying, walking, pushing, pulling, lifting, finger dexterity, grasping, feeling, talking, and hearing, sometimes exerting in excess of 20-100 pounds of force.  The worker is required to have close visual acuity to perform daily activities.  May be subject to inside environmental conditions, hazards (including electrical currents and exposure to high heat and chemicals), infectious diseases, and may be required to function around prisoners and mental patients.
 
Skills:  Successful completion of Basic Cardiac Life Support Course C.  The PT must possess demonstrated knowledge, training, and skills necessary and appropriate to the neonate, child, adolescent, adult and older adult.  They must demonstrate communication and interpersonal skills, the ability to deal effectively with multi disciplinary team members, patients, patient’s family members and physicians.  Must possess effective decision making, evaluation/assessment and critical thinking skills.  Supervisors and acting supervisors must be able to set priorities and effectively delegate/coordinate multi disciplinary care based on the needs of the patient, the census, staffing and department standards/protocols.
